The organizational landscape of health care delivery  is being rearranged. New managed models  also known as  intergrated service networks and healthcare providers. This organized delivery system  combines organization,  financing, and delivery of healthcare  to our patients(pt). The organized delivery system as a network of organization provides a coordinated continuum of services to as clinical and fiscal accountability for the people around insurance companies. The system  has full or partial ownership involvement  with   HMO , or PPO model  and  properly managed clinical affairsr responsible  to provide leadership, development of clinical outcome measures, quality care for pts, and better  financial performance.Ref. <!--filtered-->http://content.healthaffairs.org/cgi/content<!--filtered-->

The changes in the delivery of healthcare models have emerged due to research and development of more advanced treatment solutions that are also being conducted in more cost-efficient methods. These technological advances have allowed service-providers to identify the causes of the illnesses more accurately and provide care quicker.  Hospitals are able to treat more patients faster and have a better turnover on beds which helps the bottom line for administrators by being able to treat patients faster.  The evolution of healthcare delivery also helps prevent deaths and preventable illnesses due to non-treatment with this expediency in treatment.  Healthcare providers have also been able to streamline treatment by establishing outpatient surgery centers for the less risky and elective types of surgeries, which decreases the workload of physicians and staffers at hospitals.  These innovations allow for the proper number of surgical and treatment procedures that provide the best in patient safety.  Pharmaceutical companies have also played an essential part of this evolution through their research and development of drugs to treat illnesses that have not been previously treatable.

 

The evolution of models of health care delivery resulted in numerous positive clinical and financial outcomes over the years. Some people despise change because change is uncomfortable, it requires that we work out of our comfort zone. Some of the positive clinical outcomes that occurred over the years were due to a number of factors to include trial and error. Change was brought about by an increase in consumers seeking medical services. This created a need for more professionals in this field of study which led to the availability of more specialists to more effectively and efficiently treat patients. "End stage renal disease centers began as a specialized service within hospitals but increasingly were provided by independent, freestanding units, an instance of the multiple and diverse services provided by a diffuse form becoming 'unbundled' and allocated to specialist organizations." (Mick & Wyttenbach, 2003, para. 10)  More educated consumers over the years meant that the standards of care were adapted to meet the expectations of those served. The Joint Commission was established to maintain uniform standard of operations among facilities. The health care financial outcomes that resulted from the evolution of health care delivery included that benefits paid out to Medicare participants increased over the years to allow for more procedures and participants to be covered. Increasing medical expenses helped to support research and modern medical equipment that directly benefit consumers in the long run.

I think that the healthcare organizations have good delivery methods that are being arranged in new ways.  I think that these methods are great and need to be honed to make sure that they are used properly.  The methods that are being used have integrated themselves into healthcare avenues that can be found in healthcare providers. These providers have the market on what healthcare is and what direction healthcare will go.  having new methods also includes financial goals that need to be addressed.  The system  has full or partial ownership involvement with HMO's or PPO model and properly managed through clinical trials so that they can provide leadership to those that are looking for outcomes measures and better performance than anything that is out there in the business world.  Systems are being changed all the time and it is hard to keep up with them especially in healthcare.

To me the most dynamic demonstration of the evolution of health care delivery is the expansion of specialists. Doctors no longer feel they need to treat every patient that comes to them for care. Referrals are now common place. Cardiologists may refer a patient with PAT to another cardiologist, while they may specialize in heart value issues. This specialization has improved the early diagnosis and effective treatment of people prolonging life. This came at a price. It has become lucrative for the specialist who can demand higher payments for their specialized knowledge and experience. So the change from general practitioners to specialists has improved the outcomes for patients, while it increased profits for doctors who successfully specialized in a high demand category.

The development of the health care industry of the United States is a process of continual evolution.  For example, many managed care plans have relaxed their constraints, currently allowing members to use providers outside the network. Another positive result under contract to provide health care coverage for the agreed range, HMOs assume the financial risk and gain of the enterprise, relieving the consumer from this burden. The evolution has provided improved patient-focused, quality care.

The delivery of health care in the United States has evolved through a series models, with each model resulting in some positive clinical and financials outcomes. Before the inception of the managed care model, physicians enjoyed some level of cultural authority, where independent practice was dominant (Mick & Wyttenbach, 2003). The focus during this period was quality of care. As health care expenditures began to rising, making the fee-for-service a burden for patients, managed care was initiated as a means to control costs. Congress passed the Medicare and Medicaid programs in 1965 to provide public funding for health care services to the elderly and the indigent (Mick & Wyttenbach, 2003). Regulatory agencies were created to oversee the delivery of health care. The delivery of care evolved to into a market based system where health care providers were made to compete for price and services. This has created some level of efficiency in the delivery of care, though costs still remain an issue, especially in Medicare where seniors are made to pay out of pocket when they reached the threshold (doughnut hole) of the amount covered for prescription drugs.

Wow, positive outcome let me think about this one. From our readings, “the integrated delivery system, received critically important moral justification by the professional associations that first proposed the concept (American Hospital Association, 1990; 1992; Catholic Health Association, 1992)” (Scott, 2003). What this did, it connected loose ends within the health care system to provide better care by way of a whole care system. As our readings also stated, and why I believe it is a positive, the health care organizations are more involved in the communities in which they serve with the outcome of a healthier community. Also, health care organizations are working together to provide better care for the patient. One example of a positive, two different health care organizations working together and integrating electronic medical records, to share patient information in a time of an emergency.
